(STEP 1) For a new applicant applying of C.O should be submitted a copy each of the following documents:-

1. Letter of specimen signature who were authorized to sign on the application of C.O
2. Company Registration Certificate. (Latest Form 24, 44, 49 & M&A – Certified by Company Secretary)
3. Photo copy I.C (For those authorized signatories)
4. Statutory Declaration
5. Inspection Fees RM500/=

(STEP 2) The application must be accompanied with a copy of each document and to be submitted as stated below.

1. Duly completed certificate of origin form.
2. Invoice on purchasing of materials & products.
3. Packing List
4. Bill of Lading
5. Custom Declaration Form (K2)
6. Letter of Indemnity
